<image:caption>Sometimes there is an extra piece of bone on the inside of the foot where the posterior tibialis tendon attaches to the navicular. This is quite common (8% of population). Although most patients do have any symptoms, some may experience pain in the area and may require specialist imaging. In itself it does not cause a flat foot. Surgery may sometimes be required if symptoms persist and generally involves removal or re-attachment of the extra bone.</image:caption>

If non-operative treatments are not sufficient then there are very effective surgical options. State-of-the-art modern keyhole surgery techniques, pioneered in the UK by Mr Redfern, can be helpful to ‘tidy up’ spurs and ridges of bone from the joint which are often responsible for pain and restriction in joint movement.</image:caption>

<image:title>Minimally Invasive Lesser Toe Surgery - Hammer Toes</image:title>
<image:caption>This is perhaps the commonest type of toe problem that Mr Redfern sees in his practice. It often affects the second toe as seen here. It is usually but not always, due to a bunion (hallux valgus) upsetting the mechanics of the forefoot. The hammer deformity occurs due to overload of the joint at the base of the toe which damages the restraining ligaments.</image:caption>

<image:caption>This diagram shows how a tiny burr is used to cut a slot in the bone(s) of the toe and the toe then re-aligned by bending the remaining hinge of bone. This can be repeated in different bones of the same to correct the alignment in all 3 planes without resorting to permanently stiffening (fusing) the joints.</image:caption>

Arthritis of this joint is strongly inherited and usually affects both feet although not necessarily to the same extent. In these X-rays you can see the big toe joint looks disorganised compared to the other joints with spurs around the edges of the joint and a narrowed space between the two sides of the joint due to loss of cartilage in the joint.</image:caption>

This is all carried out using high-tech image guidance (x-ray image intensifier).</image:caption>

      <image:title>Ankle Ligament Repair Surgery - MRI scan</image:title>
<image:caption>An MRI scan is very useful in confirming torn or damaged ligaments in the ankle and is also important to ensure that there is no other damage to the nearby tendons or the ankle joint. This image shows a cross section of the ankle on MRI and the torn anterior talofibular ligament can be clearly seen. This is the most commonly injured of the ankle ligaments and failure of this ligament to heal properly can lead to symptoms of instability (ankle giving way or rolling over easily) and/or insecurity (not trusting the ankle). Sometimes the patient will also complain of pain in the ankle due to inflammation inside the joint (called synovitis). This can sometimes result from the lack of constraint of the joint which allows the lining of the joint to be ‘pinched’.</image:caption>

<image:title>Minimally invasive Hallux Rigidus Osteotomy - Hallux Rigidus</image:title>
<image:caption>Hallux rigidus is the medical term for arthritis of the joint at the base of the big toe.
